What the MCC exams cost 5
How much does the MCCQE cost, and can I get a refund if I withdraw?
The MCCQE application fee is $1,500.00. The full fee is non-refundable. In certain circumstances you may be eligible for a refund, but a $750.00 withdrawal fee is charged.
You receive a partial refund only if you have not scheduled an appointment with Prometric and then withdraw your MCCQE application or let your eligibility window expire, or if you cancel your scheduled Prometric appointment more than 120 hours before your exam appointment time and then withdraw your application or let your eligibility window expire.
If you have not scheduled your appointment, withdraw through your physiciansapply.ca account: click Examinations in the left panel, then Withdraw from an exam at the bottom of the centre panel, and follow the steps. If you have already scheduled your appointment, you must first cancel it directly with Prometric. A withdrawal request is processed within 15 business days.

How much does the NAC Examination cost, and can I get a refund?
The NAC Examination application fee is $3,320.00. The full fee is non-refundable, and full refunds of application fees are not available.
Withdrawing
If you withdraw before the deadline, you can get a partial refund, but a $538.00 withdrawal fee is charged. The MCC's Examination and service fees page calls this the withdrawal deadline; its Withdrawing page calls it the withdrawal with partial refund deadline. If you withdraw after the deadline, you forfeit your full application fee.
Being absent on exam day
The two MCC pages state this differently.
The MCC's Withdrawing page says that if you are absent on the day of the exam, you forfeit your full application fee. It does not mention an exception.
The MCC's Examination and service fees page says that if you are absent on the day of the exam due to extenuating circumstances, you may be eligible for a refund, but an administration fee of $1,660.00 would be charged.

How much does the TDM Examination cost?
TDM Examination application: $2,850.00. The full fee is non-refundable. In certain circumstances, you may be eligible for a refund; however, a $1,425.00 withdrawal fee would be charged. These application and withdrawal fees will be in effect starting with the September 2026 exam.

What is the MCCQE eligibility window, and can I extend it?
After your application is accepted, you have a 12-month eligibility window to schedule and write the MCCQE at an available Prometric test centre. Unless you extend it, the window expires after 12 months or when you write the exam, withdraw from it, or are absent from it, whichever comes first.
If you are not ready to write the MCCQE within your window, you can extend it once by an additional 12 months. You can only do this when your current window is within 60 days of expiring, and not before. The extension costs $120.00. If the MCCQE application fee has increased since you originally applied, you also pay the difference. The extension fee is non-refundable.
If you have not taken the MCCQE within your original window and did not extend it, you are automatically withdrawn and given a partial refund.

What MCC practice exams are there for the MCCQE?
The Preparatory Examination (PE) is the same length as the MCCQE. The Preparatory Exam-Lite (PE-Lite) is half the length, with a set of entirely new questions. Questions and answers are created by MCC subject matter experts, then refined and approved by physician test committees.
The MCC is also introducing a PE form for $1. It offers a unique set of 230 multiple-choice questions.

How do I share my verified credentials with CaRMS, the Royal College, the CFPC or another organization?
Through physiciansapply.ca, free of charge for organizations registered with the MCC, and through the MCC's File Transfer Service, for a fee, for any other organization.
Organizations registered with the MCC
Organizations must register with the Medical Council of Canada (MCC) to receive credentials through physiciansapply.ca. The Canadian medical organizations on the MCC's list are the Canadian Resident Matching Service (CaRMS), the Royal College of Physicians and Surgeons of Canada and the College of Family Physicians of Canada. There is no fee for sharing documents with organizations registered with the MCC.
The CaRMS match
CaRMS can transfer these documents from your physiciansapply.ca account: your Medical Council of Canada examination scores, your medical school transcript, and your diploma. Transfer the diploma only if you do not already have a diploma in the Document Tracking section of your CaRMS Online account.
Organizations not registered with the MCC
You can still give them access to documents in the repository through the File Transfer Service. It covers source-verified medical credentials, the Educational Credential Assessment report, individual examination results, and the Registration and examination summary. A fee applies per document. If you transfer more than 4 documents to the same third party in a single transaction, you are charged for only 4. The organization receives an email when the files are transferred and must access them within 30 days.

Can I apply to a provincial regulator through physiciansapply.ca, and what does it cost?
Yes, to the regulators set up for it. Through your physiciansapply.ca account you can apply to one or more of the medical regulatory authorities (MRAs) that are set up to receive applications through the portal, which is a majority of MRAs. You can use it only to apply for independent registration for the first time in a jurisdiction.
The application for medical registration costs $240.00. Refunds can be processed only with the approval of the appropriate MRA, and you should contact that MRA directly. If a refund is approved, the MCC charges a $70.00 administrative fee.
Your answers to any MRA-specific questions are seen only by that MRA. Each MRA sees only your application for its own jurisdiction and will not know if you applied elsewhere.

Do I need a job offer or sponsor for a clinical assistant or associate physician licence?
In each province below, the licence follows a position with an employer or sponsor.
- Alberta (associate physician): send a completed prescreening application to [email protected]. Successful applicants can then apply for associate physician positions with Alberta Health Services (AHS) or Covenant Health, and registration follows a letter of offer and completion of the orientation program.
- Saskatchewan (clinical assistant): the Saskatchewan Health Authority decides independently when to open a program for limited-roles licensure, and sponsors the candidates.
- Manitoba (clinical assistant): an offer of employment is not required to apply, but registration cannot be granted without one. Applications stay valid for six months.
- New Brunswick (clinical assistant): a regional health authority, Horizon or Vitalité, must sponsor you for employment within a CPSNB-approved clinical assistant program. Applications go through physiciansapply.ca with a $450 non-refundable fee.
- Nova Scotia (clinical assistant): Nova Scotia Health and IWK Health employ clinical assistants. You need a letter of offer in an approved physician extender program with one of them before the licence is issued.

What is the LMCC, what are the requirements, and do I need to apply and pay for it?
The Licentiate of the Medical Council of Canada (LMCC) is granted by the Medical Council of Canada (MCC). It is part of the requirements set by the provincial and territorial medical regulatory authorities (MRAs) for issuing a licence to practise medicine in their jurisdictions.
Requirements
The MCC says that as of January 26, 2026, to be eligible for inclusion in the Canadian Medical Register a graduate in medicine must:
- be a graduate of a medical school accredited by the Committee on Accreditation of Canadian Medical Schools, or of a medical school listed in the World Directory of Medical Schools with a sponsor note indicating it is an acceptable medical school in Canada, or of a United States School of Osteopathic Medicine accredited by the American Osteopathic Association;
- have been awarded a pass result on the MCC Qualifying Examination (MCCQE), formerly the MCCQE Part I;
- have the required medical credentials, including the medical degree or diploma, successfully source verified through the MCC or, in exceptional circumstances, have provided evidence of the required medical credentials that is deemed acceptable by the executive director and CEO;
- have paid the LMCC application fee to the MCC and not have any outstanding fees payable to the MCC.
Applying and paying
It depends on when you applied for the MCCQE.
- Applied for the MCCQE after January 26, 2026: the LMCC is issued automatically at no additional cost. There are no additional steps, fees or follow-up required.
- Applied for the MCCQE before January 26, 2026: log in to your physiciansapply.ca account, click Other Service Requests, select Apply for LMCC, and pay the LMCC application fee as well as any other outstanding fees to the MCC. The LMCC application fee is $50.00 and is non-refundable.

How do I get an ECA from the MCC, what does it cost, how long does it take, and how long is it valid?
$130.00 a copy, non-refundable. Your medical degree has to be through source verification first. The MCC puts the report in your physiciansapply.ca account within 14 business days of your request, and it is valid for immigration purposes for 5 years.
What you need before you can request one
An international medical graduate can request an ECA report from the MCC on meeting one of two options.
- Option one: your final medical degree or diploma has been source verified by the MCC with a status of verification returned - passed.
- Option two: your medical school transcript has already been source verified by the MCC with a status of verification returned - passed, and that transcript confirms that the degree was awarded and the date it was awarded, and your final medical degree or diploma has been submitted for source verification and has a status of sent for verification or resent for verification.
What it costs
Educational Credential Assessment: $130.00 per copy. The fee is non-refundable.
How long it takes, and how IRCC gets it
The MCC generates your ECA report and makes an electronic copy (PDF) available in your physiciansapply.ca account within 14 business days of your request. Allow up to 8 weeks for delivery by regular mail, depending on your mailing address. Once your ECA report is in your physiciansapply.ca account, it is accessible to Immigration, Refugees and Citizenship Canada.
How long it is valid
Your ECA report is valid for immigration purposes for 5 years from the time it is issued. You can request a new ECA report, if needed, 4 years following your original report date.

How much do Royal College assessments and exams cost?
The assessment fee depends on the route, and exam fees are paid separately.
Practice Eligibility Route (PER)Its Assessment and exam fees page (effective January 1 to December 31, 2026) lists "Practice Eligibility Route (PER) C$4,965"." Fees are due when you apply for assessment.
Approved Jurisdiction Route
Assessment of training in an approved jurisdiction is listed at C$4,965 for a specialty and C$2,020 for a subspecialty or higher specialty training (2026 fees).
SEAP and PER-SEAP
The 2026 assessment fees list the Subspecialty Examination Affiliate Program (SEAP) at C$1,980 and the Practice Eligibility Route for the Subspecialty Examination Affiliate Program (PER-SEAP) at C$4,645.
Exam fees
Exam fees (effective January 1 to December 31, 2026) are C$2,565 for a spring or fall written exam, C$2,565 for a spring or fall applied exam, and C$5,130 for a spring or fall comprehensive objective exam. You pay your exam fees when you register, and late registration is not accepted.

What are the PER application deadlines, and what happens if I apply late?
Applications are accepted anytime, but each exam session has a specific deadline:
- Spring exam: apply by April 30 of the year before the exam.
- Fall exam: apply by August 31 of the year before the exam.
Priority
Priority Level 1 applies to in-season applications submitted within the primary intake period (for example, fall or spring). Priority Level 2 applies to off-season applications, submitted outside peak intake periods. Applications submitted after the deadline for a future exam cycle will not incur late fees and will be assessed as Priority Level 2.
Late applications
Applications received after the deadline are subject to late fees: C$810 if less than 60 days after the deadline and C$1,655 if more than 60 days after. Paying the late fee does not guarantee that the Royal College will assess your application in time to register for the next administration of the exam. If applications received after the deadline for the upcoming exam are not processed in time to register for it, they may be pushed to the following exam year.

How much is the CFPC exam for practice-eligible candidates, and what is refundable?
The practice-eligible fee is $5,972 CAD for Fall 2026 and $6,091 CAD for Spring 2027 and Fall 2027 (residency eligible: $4,478 and $4,568). A tuition tax credit is available to each candidate who attempts the examination.
Non-refundable component
Beginning July 1, 2025, a non-refundable component of the application fee of $500 applies to all new practice-eligible applicants. It is part of the exam fee, not an additional cost. The CFPC states: "This component of the exam fee is non-refundable and retained only when an application is submitted and rejected due to incomplete application information or the absence of supporting documentation".
Cancellation
Withdrawal more than 60 days before the exam is refunded less a non-refundable $500 administration fee. Within 30 to 60 days, a penalty of 50 per cent applies. There is no refund within 30 days. Fees paid for a previous attempt are not transferable to a later session.

How do I apply for the CFPC Equivalent Jurisdictional Pathway, what does it cost, and what must be source verified?
Steps
- Complete the online self-assessment questionnaire.
- Apply to become a member of the CFPC.
- Complete the online application and pay the $533 non-refundable fee.
- Register your credentials with the Medical Council of Canada's Physician Credentials Repository and share them with the CFPC.
Fees
Effective March 1, 2026, the CFPC's Board of Directors approved an increase of the non-refundable application fee to $533 and the certification fee to $3,518. The application steps say to "Pay the processing fee of $3,518 once all the requirements are met."
Source verification
The CFPC requires source verification of:
- the medical degree;
- postgraduate family medicine training;
- certification in family medicine;
- a Certificate or Letter of Good Standing from the licensing body, not older than six months;
- a Letter of Good Standing from the certifying body, not older than six months.

What is the CLASS program and what does it cost?
The CLASS program is intended for physicians wishing to pursue certification through the practice-eligible route; participants complete one Linking Learning to Practice exercise and one Professional Learning Plan. Practice-eligible candidates who fail to complete the CLASS program will have their eligibility to take the Examination withdrawn. The CLASS program fee is $1,261 CAD.

What does applying through CaRMS cost, and can I get a refund?
You pay a registration fee for the match, plus a fee for each program you apply to. None of it is refundable.
What you pay for
All applicants pay a registration fee to take part in a CaRMS match, as well as program application fees. CaRMS is a not-for-profit, fee-for-service organization, and it sets its fees on a cost-recovery basis, which means it charges only what it costs to provide its services. CaRMS says the fees pay for the support and service it gives every applicant who goes through a CaRMS match, such as customized presentations to help applicants prepare for their match year.
Refunds
There are none. CaRMS says all fees for registration and application are non-refundable, including those received from applicants who later withdraw or go unmatched. For more information it points to [email protected].

What does the second iteration of the R-1 match cost?
The R-1 match fee covers your participation in both the first and the second iteration, and includes applications to four programs.
Any of those four included program applications you did not use in the first iteration carry over to the second iteration. They do not carry over from one match year to the next.

What happens if my PER application is found ineligible?
The Royal College's support team explains the reason in your ruling letter, indicating the additional residency training required to meet the Royal College's specialty-specific training requirements or training experiences.
The assessment fee
Assessment fees submitted with an application are non-refundable. Separately, the PER page and PER policy state that incomplete applications "will be withdrawn, and assessment fees will be refunded."
Reconsideration
A reconsideration may be initiated if substantive new information about your credentials or training is received, or if there was an error in the administration of policy; for example, completing two (2) years in a Canadian accredited program in the specialty under the supervision of a Program Director. The Royal College adds: "if an applicant completed two (2) years of residency in their home country, this may also be considered as substantive new information subject to review." The decision of the Executive Director, Standards and Assessment, or their delegate, is final and not subject to appeal.

Can I defer or withdraw from a Royal College exam, and what does it cost me?
Deferral
The Royal College will consider deferral of examination eligibility for one (1) additional year if you cannot attend because of a serious accident, illness or other impediment. Deferrals will generally not be granted for missed flights and other travel delays, minor illness, lack of preparedness, scheduling conflicts, or delays in obtaining citizenship or visas.
Withdrawal or not showing up
If you withdraw from your examination, or do not appear at the exam, you will have used one year of your five years of exam eligibility, unless you were granted a deferral.
Refunds
Notice of withdrawal at least 20 business days before the examination results in a refund of 50 per cent of the examination fees. Notice less than 20 business days before the examination results in no refund.

How do I ask for a reconsideration or appeal of my MCCQE result?
Send the MCC a message through your physiciansapply.ca account within 15 calendar days of receiving your exam result. You are not eligible for reconsideration if you received a pass result, or if you did not report the incident directly to the MCC within 7 days of your exam. You cannot request a reconsideration based on the content of the exam.
A pass or additional marks cannot be granted through reconsideration. The process is expected to take about 11 weeks from when you receive your result to when you receive the decision.
If you are eligible to reapply for the MCCQE, you do not need to wait for the outcome of the reconsideration before applying to retake the exam.
Once you receive a reconsideration decision, you may appeal it by sending the MCC a message through your physiciansapply.ca account within 30 calendar days of the date on the reconsideration decision letter. A pass or additional marks cannot be granted through the appeal process either.

Can I have my certification exam re-marked or reviewed?
Neither college re-grades your exam. Each offers paid reviews, with its own deadlines.
The Royal College
Formal reviews "are conducted to investigate alleged, significant procedural irregularities in the assessment process. They are not re-grading or re-scoring exercises." You must "notify the Office of the Executive Director of Standards and Assessment of the Royal College in writing within 30 days of the date appearing on your Summary of Performance". The Assessment and exam fees page lists "Formal reviews C$1,390".
The CFPC
Candidates may request a review of marks within 21 calendar days of the results disclosure letter ($250); it does not include rereading or regrading. A reconsideration of awarded standing for a significant procedural error must be received within 31 calendar days ($300, non-refundable), and a fail cannot become a pass.
